California men allegedly billed Medicare $3.5m through fake medical firms

Nouman Mustafa and Mohsin Khan have been charged with healthcare fraud for filing false claims to Medicare through fictitious businesses that lacked physical locations for medical supplies. They reportedly sourced personal data from legitimate Medicare beneficiaries via foreign contacts. To avoid detection, they frequently changed company identities, leading to arrests after a federal probe, encompassing charges of identity theft and healthcare fraud.
